ABSTRACT

Introducción. Una fístula es una conexión anormal entre dos superficies epitelizadas. Cerca del 80 % de las fístulas entero-cutáneas son de origen iatrogénico secundarias a cirugía, y un menor porcentaje se relacionan con traumatismos, malignidad, enfermedad inflamatoria intestinal o isquemia. La morbilidad y las complicaciones asociadas pueden ser significativas, como la desnutrición, en la que intervienen múltiples factores. Métodos. Se realizó una búsqueda de la literatura en las bases de datos de PubMed, Google Scholar y SciELO, utilizando las palabras claves descritas y se seleccionaron los artículos más relevantes de los últimos años. Resultados. La clasificación de las fístulas se basa en su anatomía, su gasto o secreción diaria y su localización. Existe una tríada clásica de las complicaciones: sepsis, desnutrición y anomalías electrolíticas. El control del gasto de la fístula, el drenaje adecuado de las colecciones y la terapia antibiótica son claves en el manejo precoz de estos pacientes. Los estudios recientes hacen hincapié en que la sepsis asociada con la desnutrición son las principales causas de mortalidad. Conclusiones. Esta condición representa una de las complicaciones de más difícil y prolongado tratamiento en cirugía abdominal y colorrectal, y se relaciona con importantes tasas de morbilidad, mortalidad y altos costos para el sistema de salud. Es necesario un tratamiento multidisciplinario basado en la reanimación con líquidos, el control de la sepsis, el soporte nutricional y el cuidado de la herida, entre otros factores.


Introduction. A fistula is an abnormal connection between two epithelialized surfaces. About 80% of enterocutaneous fistulas are of iatrogenic origin secondary to surgery, and a smaller percentage are related to trauma, malignancy, inflammatory bowel disease or ischemia. The associated morbidity and complications can be significant, such as malnutrition, in which multiple factors intervene. Methods. A literature search was carried out in the PubMed, Google Scholar and SciELO databases using the keywords described and the most relevant articles from recent years were selected. Results. The classification of fistulas is based on their anatomy, their daily secretion output, and their location. There is a classic triad of complications: sepsis, malnutrition and electrolyte abnormalities. Control of fistula output, adequate drainage of the collections and antibiotic therapy are key to the early management of these patients. Recent studies emphasize that sepsis associated with malnutrition are the main causes of mortality. Conclusions. This condition represents one of the most difficult and prolonged complications to treat in abdominal and colorectal surgery, and is related to significant rates of morbidity, mortality and high costs for the health system. Multidisciplinary treatment based on fluid resuscitation, sepsis control, nutritional support, and wound care, among other factors, is necessary.

Introducción. Una de las principales intervenciones que realiza el profesional de enfermería es el cuidado de las heridas, su finalidad es evitar posibles complicaciones y estimular la cicatrización del tejido lesionado. El objetivo de esta investigación fue determinar las intervenciones de enfermería realizadas en pacientes con abdomen abierto y fístulas enterocutáneas mediante la revisión de literatura. Metodología. Se realizó una revisión narrativa en un lapso de 10 meses. Las bases de datos utilizadas fueron: PubMed, ScienceDirect, ClinicalKey, MEDLINE, SciELO y Ovid. Se realizó revisión de 50 artículos que cumplían con el objetivo de la revisión. Resultados. Se encontraron cuidados de enfermería relacionados con los cuidados básicos y específicos en pacientes con abdomen abierto y en fístulas enterocutáneas. Discusión. Diversos autores coinciden en que el uso de la Bolsa de Bogotá, es una de las intervenciones de enfermería más utilizadas para las heridas de abdomen abierto, la medición intraabdominal, que permite la identificación precoz del Síndrome Compartimental Abdominal, y el mantenimiento del objetivo nutricional en pacientes con fístulas enterocutáneas, permiten una recuperación exitosa. Conclusiones. Se pudo determinar que existen diversos cuidados de enfermería que deben considerarse en el manejo del abdomen abierto y de las fístulas enterocutáneas, todos estos cuidados permiten que el personal de enfermería tenga un soporte científico útil en el momento de brindar un cuidado a los pacientes con estas condiciones de salud. Esto, con el fin de evitar o disminuir los riesgos de deshidratación, desequilibrio hidroelectrolítico, desnutrición, sepsis e incluso la muerte.


Introduction. One of the main interventions carried out by nursing professionals is the care of wounds. Its purpose is to avoid possible complications and stimulate the scarring of the wounded tissue. The objective of this investigation was to determine the nursing interventions carried out in patients with open abdomens and enterocutaneous fistulas via a literature review. Methodology. A narrative review was carried out over 10 months. The databases used were: PubMed, ScienceDirect, ClinicalKey, MEDLINE, SciELO, and Ovid. Fifty articles that met the objective of the review were used. Results. Nursing care related to basic and specific care in patients with open abdomens and enterocutaneous fistulas was found. Discussion. Different authors agree on the use of the Bogotá Bag, is one of the most used nursing interventions for open abdominal wounds, intraabdominal measurement, which allows for early identification of Abdominal Compartment Syndrome, and maintenance of nutritional objectives in patients with enterocutaneous fistulas allow for successful recovery. Conclusions. We could determine that there are different types of nursing care that must be considered in the handling of open abdomen and enterocutaneous fistulas. All these types of care allow for nursing staff to have a useful scientific support when providing care to patients with these health conditions. This is in order to avoid or reduce the risks of dehydration, hydroelectrolytic imbalance, malnutrition, sepsis, and even death.

Uma das principais intervenções realizadas pelo profissional de enfermagem é o cuidado de feridas, sua finalidade é evitar possíveis complicações e estimular a cicatrização do tecido lesado. O objetivo desta pesquisa foi determinar as intervenções de enfermagem realizadas em pacientes com abdome aberto e fístulas enterocutâneas por meio de uma revisão de literatura. Metodologia. Foi realizada uma revisão narrativa durante um período de 10 meses. As bases de dados utilizadas foram: PubMed, ScienceDirect, ClinicalKey, MEDLINE, SciELO e Ovid. Foi realizada uma revisão de 50 artigos que atenderam ao objetivo da revisão. Resultados. Foram encontrados cuidados de enfermagem relacionados aos cuidados básicos e específicos em pacientes com abdome aberto e fístulas enterocutâneas. Discussão. Vários autores concordam que o uso da Bolsa de Bogotá, é uma das intervenções de enfermagem mais utilizadas para feridas abertas abdominais; a medida intra-abdominal, que permite a identificação precoce da Síndrome do Compartimento Abdominal; e a manutenção do objetivo nutricional nos pacientes com fístulas enterocutâneas, permitem uma recuperação bem sucedida. Conclusões. Foi possível determinar que existem diversos cuidados de enfermagem que devem ser considerados no manejo de abdome aberto e fístulas enterocutâneas, todos esses cuidados permitem que a equipe de enfermagem tenha um apoio científico útil no momento de prestar cuidados aos pacientes com estas condições de saúde. Isso, a fim de evitar ou reduzir os riscos de desidratação, desequilíbrio hidroeletrolítico, desnutrição, sepse e até mesmo a morte.

ABSTRACT

Se realizó una revisión sistemática de la literatura científica en relación con las variantes de tratamien-to que recibieron las piezas dentarias permanentes asociadas a la presencia de una fístula cutánea. Se incluyeron reportes de casos clínicos en inglés, pu-blicados en los últimos diez años. Se obtuvieron 481 artículos, 359 excluidos por escasez de datos en títu-lo e incumplimiento de criterios de selección. Se ex-cluyeron 89 por carecer de tratamiento. Se incluye-ron 33 artículos. Se analizaron fístulas de 45 piezas dentarias, superiores e inferiores, anteriores y pos-teriores. En base a los resultados obtenidos se llegó a la conclusión de que las fístulas cutáneas faciales de origen odontogénico se diagnostican de manera errónea; esto conduce a un tratamiento inadecuado y secuela estéticas. Realizar un correcto diagnóstico y adecuado plan de tratamiento permite una curación rápida y predecible. Los hallazgos obtenidos estable-cen que realizar un correcto tratamiento endodóntico en la pieza afectada, es la terapéutica adecuada para lograr el cierre definitivo de la fístula, sin necesidad de tratar quirúrgicamente la misma (AU)


A systematic review of the scientific literature was carried out in relation to the variants of treatment received by permanent teeth associated with the presence of a cutaneous sinus tract. Clinical case reports in English, published in the last ten years, were included. A total of 481 articles were obtained, 359 were excluded due to lack of title data and non-compliance with selection criteria. 89 were excluded due to lack of treatment. 33 articles were included Cutaneous sinus tracts of 45 teeth, upper and lower, anterior and posterior, were analyzed. Based on the results obtained, it was concluded that facial cutaneous sinus tracts of odontogenic origin are misdiagnosed, leading to inadequate treatment and aesthetic sequelae. Carrying out a correct diagnosis and adequate treatment plan allows a quick and predictable healing. The findings obtained establish that performing a correct endodontic treatment in the affected piece is the appropriate therapy to achieve the definitive closure of the cutaneous sinus tract, without the need to surgically treatment (AU)

Introducción: La sutura mecánica es una opción para el cierre de la faringorrafia en laringectomía total por cáncer de laringe. Objetivo: Comparar el uso de sutura mecánica lineal con sutura manual de la faringe durante la laringectomía total por cáncer de laringe. Material y Método: Se realizó un estudio de tipo experimental prospectivo. Se compararon dos grupos pacientes: Los pacientes con sutura mecánica desde 2018 a marzo de 2020 y los pacientes con sutura manual previa a enero de 2018 en el Servicio de Otorrinolaringología del Hospital Barros Luco. Se analizaron fístula faringo-cutánea posoperatoria (FFC), tiempo operatorio, estadía hospitalaria y costo. Resultados: El estudio incluyó a 14 pacientes, cada grupo con n = 7. El grupo con sutura mecánica presento 0% de FFC y el grupo sutura manual 28%. El grupo con sutura mecánica reinicio alimentación a los 7 días y el grupo con sutura manual en promedio a los 11,5 (7-23) días. (p = 0,0023). El tiempo promedio de cirugía para el grupo experimental es de 288 ± 37,4 minutos y con sutura manual 311 ± 32,4 minutos. (p = 0,0176). El promedio de hospitalización para el grupo experimental fue de 11 ± 2,6 días (9 a 16), y para el grupo control fue de 21 ± 14 días (10 a 49) (p < 0,0001). Conclusión: La sutura mecánica es un procedimiento fácil de usar y seguro. Existiría un beneficio en el uso de sutura mecánica para el cierre faríngeo al compararlo con la sutura manual al disminuir el número de FFC, el tiempo operatorio y los días de hospitalización.


Introduction: Mechanical suture is an option for the closure of the pharyngorrhaphy in total laryngectomy due to laryngeal cancer. Aim: To compare the use of linear mechanical suture with manual pharyngeal suture during total laryngectomy for laryngeal cancer. Material and Method: A prospective experimental study was carried out. Two patient groups were compared: patients with mechanical suture from 2018 to March 2020 and patients with manual suture prior to January 2018 at the otorhinolaryngology service of the Barros Luco Hospital. Postoperative pharyngocutaneous fistula (FFC), operative time, hospital stay and cost were analyzed. Results: The study included 14 patients, each group with n = 7. The group with mechanical suture presented 0% of FFC and the group with manual suture 28%. The group with mechanical suture restarted feeding at 7 days and the group with manual suture on average at 11.5 (7-23) days (p = 0.0023). The average surgery time for the experimental group was 288 ± 37.4 minutes, while for the group with manual suture was 311 ± 32.4 minutes (p = 0.0176). The mean hospitalization for the experimental group was 11 ± 2.6 days (9 to 16), and for the control group it was 21 ± 14 days (10 to 49) (p < 0.0001). Conclusion: The mechanical suture is an easy to use and safe procedure. There would be a benefit in the use of mechanical suture for pharyngeal closure when buying it with manual suture by reducing the number of FFCs, operative time and days of hospitalization.

Abstract Background: The Argon Beam Coagulator (ABC) achieves hemostasis but has potential complications in the form of argon gas embolisms. Risk factors for embolisms have been identified and ABC manufacturers have developed guidelines for usage of the device to prevent embolism development. Case report: A 49 year-old male with history of recurrent cholangiocarcinoma status post resection presented for resection of a cutaneous biliary fistula. Shortly after initial use of the ABC, the patient underwent cardiac arrest. After resuscitation, air bubbles were observed in the left ventricle via Transesophageal Echo (TEE). Conclusion: Although argon embolisms have been described more commonly during laparoscopies, this patient most likely experienced an argon gas embolism during an open resection of a cutaneous biliary fistula via the biliary tract or vein with possible transpulmonary passage of the embolism. Consequently, a high degree of suspicion should be maintained for an argon gas embolism during ABC use in laparoscopic, open, and cutaneous surgeries.


Resumo Introdução: A Coagulação por Feixe de Argônio (CFA) promove hemostasia, mas pode levar a complicações na forma de embolia por gás argônio. Os fatores de risco para embolias foram identificados e os fabricantes de aparelhos de CFA desenvolveram diretrizes para o uso do dispositivo para impedir a ocorrência de embolia. Relato de caso: Paciente masculino de 49 anos com história de colangiocarcinoma recorrente pós-ressecção foi submetido à ressecção de fístula cutâneo-biliar. Logo após o início do uso do aparelho de CFA, o paciente apresentou parada cardíaca. Após o retorno da atividade cardíaca, a Eecocardiografia Transesofágica (ETE) detectou bolhas de ar no ventrículo esquerdo. Conclusões: Embora a embolia associada ao argônio seja mais frequentemente descrita durante laparoscopia, este paciente mais provavelmente apresentou embolia provocada pelo argônio durante cirurgia aberta para ressecção de fístula cutâneo-biliar, após o argônio ganhar acesso à circulação sanguínea através das vias biliares ou da veia biliar e possível passagem do êmbolo pela circulação pulmonar. Desta maneira, deve-se suspeitar de embolia por argônio, de forma judiciosa, durante o uso de CFA em procedimento cirúrgico laparoscópico, aberto ou cutâneo.

Abstract Introduction: Pharyngocutaneous fistula is the most significant complication after salvage total laryngectomy in patients who have received previous treatment with radiotherapy with or without chemotherapy. Objective: Our purpose is to review the fistula rate in radiated patients undergoing salvage total laryngectomy, to determine if the use of pectoralis major flap interposition reduces the incidence and duration of fistula and to examine other risk factors. Methods: We made a retrospective review of patients undergoing salvage total laryngectomy for exclusively larynx cancer after failure of primary curative radiotherapy between 2000 and 2017. General data from patients, risk factors and other complications were analyzed. Results: We identified 27 patients whose mean age was 66.4 years, mainly male (92.5%). The primary closure group without pectoralis major flap included 14 patients, and the group with pectoralis major flap closure included 13 patients. Pharyngocutaneous fistula was present in 15 patients (55.5%). Global pharyngocutaneous fistula rate was higher in the group of patients without pectoralis major flap comparing with those were the flap was interposed (78.6% versus 30.8%, p = 0.047). Also the pharyngocutaneous fistulas which need to be repaired with surgery (64.3% versus 7.7%, p = 0.03) and large pharyngostomes (64.3% versus 0%, p = 0.0004) were present in a higher rate in the group closed primary without pectoralis major flap. We did not find other risk factors with statistical significance. Oral diet initiation (84 days versus 21.5 days, p = 0.039) and the duration of hospitalization (98.3 days versus 27.2 days, p = 0.0041) were much lower in patients with a preventive pectoralis major flap. Two patients died as a consequence of complications of large pharyngostomes. Conclusions: Prophylactic pectoralis major flap reduced the incidence, severity and duration of fistula and should be considered during salvage total laryngectomy.


Resumo Introdução: A fístula faringocutânea é a complicação mais significativa após laringectomia total de resgate em pacientes que receberam tratamento prévio com radioterapia com ou sem quimioterapia. Objetivo: Revisar a taxa de fístula em pacientes irradiados submetidos a laringectomia total de resgate, para determinar se o uso de interposição de retalho do peitoral maior reduz a incidência e a duração da fístula e examinar outros fatores de risco. Método: Fizemos uma revisão retrospectiva de pacientes submetidos à laringectomia total de resgate para câncer exclusivamente laríngeo após falha da radioterapia curativa primária entre 2000 e 2017. Dados gerais dos pacientes, fatores de risco e outras complicações foram analisados. Resultados: Foram identificados 27 pacientes com média de 66,4 anos, principalmente do sexo masculino (92,5%). O grupo de fechamento primário sem retalho de peitoral maior incluiu 14 pacientes e o grupo de fechamento com retalho de peitoral maior incluiu 13 pacientes. Fístula faringocutânea esteve presente em 15 pacientes (55,5%). A taxa global de fístula faringocutânea foi maior no grupo de pacientes sem retalho de peitoral maior em comparação com aqueles que receberam o retalho (78,6% vs. 30,8%, p = 0,047). Além disso, as fístulas faringocutâneas que precisaram ser reparadas através de cirurgia (64,3% vs. 7,7%, p = 0,03) e grandes faringostomias (64,3% vs. 0%, p = 0,0004) apresentaram uma taxa mais alta no grupo fechado primariamente sem retalho do peitoral maior. Não encontramos outros fatores de risco com significância estatística. O início da dieta oral (84 dias vs. 21,5 dias, p = 0,039) e a duração da internação (98,3 dias vs. 27,2 dias, p = 0,0041) foram muito menores nos pacientes com uso preventivo do retalho do peitoral maior. Dois pacientes morreram em consequência de complicações de grandes faringostomias. Conclusões: O uso profilático do retalho do peitoral maior reduziu a incidência, a gravidade e a duração da fístula e deve ser considerado durante a laringectomia total de resgate.

@#INTRODUCTION: Colo-cutaneous fistula is a very rare complication of colonic diverticular disease, occurring in approximately one percent of cases either spontaneously or after surgical or drainage procedure. Herein we describe a patient with a colo-cutaneous fistula from the sigmoid to the appendiceal stump in a post-appendectomy patient which also exits to the skin at the post-operative site. CASE PRESENTATION: The patient is a 76-year-old Filipino male who had appendectomy five months earlier and a history of diverticulitis, and presented with a subcutaneous abscess at the post-operative site. The abscess was drained, a colocutaneous fistula was radiographically established, and the surgical site was explored. Intraoperative findings showed the presence of multiple colonic diverticuli and a sigmoidcutaneous fistula on the right lower abdominal quadrant. Interestingly, an incidental descending colon mass was also noted at the splenic flexure measuring approximately 2x3 cm to which frozen section revealed adenocarcinoma which was not seen in pre-operative CT scans. An extended left hemicolectomy was performed, and no postoperative complications were noted. At present three years later, he fares well without any signs and symptoms of the disease. CONCLUSION: Diverticulitis is a common condition in the older age group that needs to be considered in patient management. Colo-cutaneous fistula may be a rare complication of the disease but should be part of our differentials as internists in working up patients presenting with persistently draining superficial wound that either occurs spontaneously or post-operatively. Patients with diverticulitis also have increased risk of colorectal cancer and diagnostic imaging may not always differentiate the two entities, thus colonoscopy should be done if possible.

Introducción. La patología umbilical es un tema frecuente en el ámbito pediátrico, la presentación clínica de una fístula umbilical hace pensar en dos patologías mencionadas en la literatura de manera independiente; la primera es la persistencia del conducto onfalomesentérico y la segunda es la persistencia del remanente del uraco producto del fracaso en el cierre de las estructuras embrionarias. Su presencia en adultos es infrecuente y no existen datos estadísticos acerca de su presentación conjunta en población pediátrica o adulta, solo algunos pocos reportes de caso. El diagnóstico se basa principalmente en la sospecha clínica, depende en gran manera del examen físico al evidenciar secreción a través del ombligo al realizar esfuerzos o maniobras de Valsalva. Objetivo. Mostrar un caso infrecuente de la presentación simultánea del conducto de uraco y onfalomesentérico en un paciente adulto. Reporte de caso. Paciente femenina de 24 años de edad con antecedentes de infecciones urinarias y celulitis periumbilicales a repetición. Se sospecha un conducto persistente onfalomesentérico por lo que es sometida a un procedimiento quirúrgico en el que se encontró incidentalmente la persistencia simultánea del conducto onfalomesentérico y persistencia del uraco. Discusión. La persistencia del conducto onfalomesentérico o la persistencia del uraco de forma individual es poco frecuente en adultos, y es aún más raro la persistencia simultánea de ambos conductos; la presencia simultánea de ambos conductos es reportada principalmente en menores de dos años. Conclusiones. La persistencia de estos conductos es rara en adultos y representa un reto diagnóstico para el clínico. Introduction. Umbilical pathology is a common topic in the pediatric sphere. The clinical presentation of an umbilical fistula leads to the consideration of two pathologies independently reported in literature. The first is a persistent vitelline duct and the second is a persistent urachal remnant as a result of the embryonic structures' failure to close. They are uncommon in adults and there are no statistical data about their presentation together in the pediatric or adult population, only very few case reports. The diagnosis is mainly based on clinical suspicion. It largely depends on a physical examination noting secretion through the navel when straining or performing Valsalva maneuvers. Objective. Show an uncommon case of the simultaneous presentation of the urachus and vitelline ducts in an adult patient. Case report. Female patient aged 24 years with a background of repeated urinary tract infections and periumbilical cellulitis. A persistent vitelline duct is suspected. Therefore, the patient is subject to a surgical procedure in which the simultaneous persistence of the vitelline duct and the urachus was found incidentally. Discussion. The persistence of the vitelline duct or the persistence of the urachus individually is uncommon in adults, and the simultaneous persistence of both ducts is even rarer. The simultaneous presence of both ducts is reported mainly in infants aged under two years. Conclusions. The persistence of these ducts is rare in adults and poses a diagnostic challenge for clinicians. Introdução. A patologia umbilical é um tópico frequente no cenário pediátrico; a apresentação clínica de uma fístula umbilical faz pensar em duas patologias mencionadas na literatura de forma independente; a primeira é a persistência do ducto onfalomesentérico e a segunda é a persistência do úraco como resultado da falha no fechamento das estruturas embrionárias. É pouco frequente sua presença em adultos e não há dados estatísticos sobre sua apresentação conjunta em população pediátrica nem adulta, apenas alguns poucos relatos de caso. O diagnóstico baseia-se principalmente na suspeita clínica, dependendo em grande parte do exame físico ao evidenciar uma secreção pelo umbigo quando realizar esforço ou manobra de Valsalva. Objetivo. Mostrar um caso infrequente de apresentação simultânea do úraco e ducto onfalomesentérico em um paciente adulto. Relato de caso. Paciente do sexo feminino, 24 anos, com histórico de infecções urinárias e celulite periumbilical recorrentes. Suspeita-se de um ducto onfalomesentérico persistente, portanto ela é submetida a um procedimento cirúrgico no qual encontrou-se a persistência do ducto onfalomesentérico e a persistência de úraco simultaneamente. Discussão. A persistência do ducto onfalomesentérico e a persistência de úraco individualmente é rara em adultos, e a persistência simultânea de ambos os ductos é ainda mais rara; esta presença simultânea é relatada principalmente em crianças menores de dois anos de idade. Conclusão. A persistência desses ductos é rara em adultos e representa um desafio diagnóstico para o profissional de saúde clínico. ABSTRACT

Abstract Introduction The main modalities of surgical treatment for laryngeal cancer include transoral laser microsurgery (TLM), open preservation surgery, and total laryngectomy (TL). In the elderly, for the presence of comorbidities, the surgical approach more appropriate in many cases remains TL. The use of a stapler for the closure of the esophagus has been introduced to reduce surgical time and postoperative complications such as pharyngocutaneous fistula (PCF). Objective In the present study, we have evaluated the effectiveness of the use of the horizontal mechanical pharyngoesophageal closure in patients who underwent TL. Methods This nonrandomized study was performed on consecutive patients with histopathologically proven squamous cell endolaryngeal carcinoma. The TLwas performed using a linear stapler to mechanically suture the pharyngotomy using the semiclosed technique. Results A total of 33 patients underwent TL, and 13 of themunderwent neck dissection. A total of 15 patients (45.4%) were ≤70 years old, and 18 were > 70 years old. Analyzing the results in relation to age, patients > 70 years old showed tumors at an earlier stage than those aged ≤70 years old. Furthermore, in this group there was a greater number of patients who had comorbidities (p = 0.014). In total, we had 2 (6%) cases of PCF in 6.6% in the group ≤70 years old, and in 5.5% of the group > 70 years old (p = 1.00). Conclusions The use of the stapler for the horizontal closure of the pharyngoesophagectomy in the patients subjected to TL is proven to be useful and safe even when used in elderly patients.

ABSTRACT

O cisto periapical tem origem associada à proliferação dos restos epiteliais de Malassez, após um processo inflamatório crônico decorrente de uma extensa lesão cariosa com acometimento pulpar. Quando um cisto periapical passa por um processo de reagudização infecciosa o quadro clínico é denominado de abscesso Fênix. O objetivo deste trabalho é relatar um caso de abscesso Fênix associado à raiz residual do dente 44. Paciente M.R.G, 19 anos, apresentou queixa principal de "espinha no meu queixo". Ao exame clínico, foi observada uma fístula cutânea na região mentual, com drenagem purulenta. Ao exame intraoral, foi observada a raiz residual do dente 44. Ao exame imaginológico, observou-se um cisto periapical associado ao dente 44 que se estendia anteriormente até a região anterior de mandíbula, fenestrando a cortical vestibular (mais espessa) e drenando para a região de mento. O paciente foi então tratado com a enucleção da lesão, seguida de plastia da fístula cutânea. Após exame histopatológico foi confirmado o diagnóstico de cisto periapical com processo de inflamação aguda. O paciente evoluiu bem após o tratamento, sem queixas álgicas. Portanto, é fundamental que o Cirurgião-Dentista conheça a etiopatogenia das lesões odontogênicas para o correto diagnóstico e tratamento em casos atípicos como o relatado


The periapical cyst origin is associated to the proliferation of epithelial cell rests of Malassez after a chronic inflammatory process due to an extensive carious lesion with pulp involvement. When a periapical cyst goes through a process of infectious exudation, clinically, it gets denominated phoenix abscess. The purpose of this study is to report a case of phoenix abscess associated to the residual root of the tooth 44. Patient M.R.G., 19 years old, presenting as main complaint a "pimple on my chin". After the clinical exam it was observed an exudating cutaneous fistula on the mentual region. After the intra-oral exam it was observed a residual root of the tooth 44. Radiographically, it was observed a periapical cyst associated to the tooth 44 wich reaches the anterior region of the jaw penetrating the vestibular cortical and draining on the mentual region. The patient has been treated with an enucleation procedure followed by a fistuloplasty. After hystological exam it was confirmed the diagnostic of periapical cyst with an acute inflammatory process. Patient has evolved well after treatment without any pain complaints. Therefore, it is fundamental that the dentists know the etiopathology of the odontogenics lesions to have a correct diagnostic and treatment of unusual cases as same as the reported.

Abstract Introduction: The pectoralis major flap is a reconstructive option to consider in the treatment of pharyngocutaneous fistula after a total laryngectomy. There are not large studies assessing variables related to pharyngocutaneous fistula recurrence after removal of the larynx. Our objectives were to review the results obtained with this type of treatment when pharyngocutaneous fistula appears in laryngectomized patients, and to evaluate variables related to the results. Methods: We retrospectively reviewed our results using either a myocutaneous or fasciomuscular pectoralis major flap to repair pharyngocutaneous fistula in 50 patients. Results: There were no cases of flap necrosis. Oral intake after fistula repair with a pectoralis major flap was restored in 94% of cases. Fistula recurrence occurred in 22 cases (44%), and it was associated with a lengthening of the hospital stay. Performing the flap as an emergency procedure was associated with a significantly higher risk of fistula recurrence. Hospital stay was significantly shorter when a salivary tube was placed. Conclusions: The pectoralis major flap is a useful approach to repair pharyngocutaneous fistula. Placing salivary tubes during fistula repair significantly reduces hospital stay and complication severity in case of pharyngocutaneous fistula recurrence.


Resumo Introdução: O retalho do músculo peitoral maior é uma opção a ser considerada no fechamento de fístula faringocutânea pós-laringectomia total. Não há grandes estudos que avaliem as variáveis relacionadas à recorrência da fístula faringocutânea após esse procedimento. Nossos objetivos foram avaliar os resultados obtidos com esse tipo de tratamento em pacientes laringectomizados com fístula faringocutânea e as variáveis relacionadas aos resultados. Método: Revisamos retrospectivamente os nossos resultados em 50 pacientes nos quais um retalho miocutâneo ou fasciomuscular do músculo peitoral maior foram utilizados para reparar a fístula faringocutânea. Resultados: Não houve casos de necrose de retalho. Após o reparo da fístula com um retalho do músculo peitoral maior, a ingestão oral foi restaurada em 94% dos casos. Houve recorrência da fístula em 22 casos (44%), a qual foi associada à duração da hospitalização. O uso do retalho como procedimento de emergência foi associado a um risco significativamente maior de recorrência da fístula. A permanência hospitalar foi significativamente menor quando utilizado um tubo de derivação salivar. Conclusões: O uso do retalho do músculo peitoral maior é uma abordagem útil para reparar a fístula faringocutânea. A colocação de tubos de derivação salivar durante o reparo da fístula reduz significativamente o tempo de hospitalização e a gravidade das complicações em caso de recorrência da fístula faringocutânea.

ABSTRACT

Orocutaneous fistulas, or cutaneous sinuses of odontogenic origin, are uncommon but often misdiagnosed as skin lesions unrelated to dental origin by physicians. Accurate diagnosis and use of correct investigative modalities are important because orocutaneous fistulas are easily confused for skin or bone tumors, osteomyelitis, infected cysts, salivary gland fistulas, and other pathologies. The aim of this study is to present our experience with a patient with orocutaneous fistulas of odontogenic origin presenting as recurrent pyogenic granuloma of the cheek, and to discuss their successful treatment.

ABSTRACT

Se presenta el caso de una paciente mujer de 53 años procedente de Piura con un tiempo de enfermedad de 20 meses, caracterizado por dolor en sitio de herida quirúrgica por colecistectomía abierta realizada hace 3 años, asociado a presencia de una masa en dicha zona, posteriormente se absceda y comienza a drenar secreción alimentaria. Los estudios de imágenes revelaron una masa dependiente de colon transverso en contacto con estómago y pared abdominal que presentaba fistulas hacia piel. Dicha masa fue extraída durante la cirugía con resultado anatomopatológico de adenocarcinoma mucinoso de colon. El caso representó un reto diagnóstico para el equipo médico y en vista de las diversas manifestaciones clínicas del cáncer de colon, sugerimos mantenerlo presente como diagnóstico diferencial en cuadros de absceso de pared abdominal y fistulizaciones entéricas.


We present the case of a 53-year-old woman with a time of illness of 20 months, characterized by pain at the site of surgical intervention for opened cholecystectomy 3 years ago, associated with a presence of a mass in said area, with the following abscess formation and fistulization of food content. Imaging studies revealed a mass dependent of the transverse colon, in contact with stomach and abdominal wall and presenting fistulas to the skin. Said mass was extracted during surgery with anatomopathological result of mucinous colon adenocarcinoma. The case represented a diagnostic challenge for the medical team and in view of the variaty of clinical manifestations of colonic cancer, we suggest that it should be consider as a differential diagnosis in cases of abdominal wall abscess and enteric fistulas.

RESUMEN Introducción: la fístula faringocutánea es la complicación postoperatoria más frecuente de la laringectomía total. Incrementa la morbimortalidad, demora la administración del tratamiento oncológico y repercute en la esfera psicológica del paciente. Objetivo: conocer el comportamiento de la fístula faringocutánea en pacientes con laringectomía total. Materiales y métodos: se realizó un estudio observacional, descriptivo, retrosprospectivo en el Hospital Universitario "Comandante Faustino Pérez", en el período comprendido entre enero 2010 a diciembre 2015. El universo lo constituyó 143 pacientes laringectomizados. Se estudiaron las variables: edad, sexo, estado nutricional previo a la cirugía, estadio del tumor, radiaciones, traqueostomía; manejo del cuello, previo a la cirugía, y evolución de la fístula. Resultados: presentó fístula postoperatoria el 28 %, predominando el sexo masculino, en un 92.3 %; y los normopesos en un 56,6 %. Se encontraban en estadio IV, el 70 %. Fueron irradiados 72,5 % y se les practicó traqueostomía, previa a la cirugía, al 95 % de los pacientes. Vaciamiento cervical se realizó al 37,5 %. En el 70 % de los pacientes, la fístula apareció entre los 8 y 14 días. Conclusiones: la incidencia de la fístula faringocutánea es mayor en el sexo masculino entre los 60-69 años. La mayor incidencia ocurrió en el año 2015. El estadio avanzado del tumor, la presencia de traqueostomía, el estado nutricional e irradiación previa, fueron los factores que más incidieron en la aparición de fístula faringocutánea. En la mayoría de los pacientes el cierre fue espontáneo con buena su evolución (AU).


ABSTRACT Introduction: pharyngocutaneous fistula is the most frequent post-surgery complication of the total laryngectomy. It increases morbimortality, delays the administration of the oncological treatment and rebounds in the patient's psychological sphere. Objective: to know the behavior of the pharyngocutaneous fistula in patients with total laryngectomy. Materials and methods: a retrospective, descriptive, observational study was carried out in the University Hospital "Comandante Faustino Pérez", in the period from January 2010 until December 2015. The universe were 143 patients who undergone a laryngectomy. The studied variables were age, sex, nutritional status before the surgery, tumor stage, radiations, tracheotomy, neck management before surgery, and fistula evolution. Results: 28 % of the patients presented post-surgery fistula, predominantly among males (92.3 %). Normal weight patients were 56.6 %; 70 % were in the IV stage. 72.5 % of the patients were irradiated and 95 % undergone tracheotomy before the surgery. 37.5 % of them undergone cervical resection. In 70 % of the patients, the fistula appeared after 8-14 days. Conclusions: the incidence of the pharyngocutaneous fistula is higher in the male sex and the 60-69 years age-group. The highest incidence occurred in 2015. The advanced stage of the tumor, the presence of tracheotomy, the nutritional status and previous irradiation were the factors that had more incidences on pharyngocutaneous fistulae. The closure of the fistula was spontaneous in most of the patients, with a good evolution (AU).

ABSTRACT

A uterocutaneous fistula is rarely reported clinical condition after uterine procedures. Many diagnostic and management strategies are being suggested. In this case report, uterocutaneous fistula after pelviscopic myomectomy was diagnosed simply with hystero-salpingo contrast sonography and managed by surgical tract excision without hysterectomy and uterine wall dehiscence repair combined with medical treatment using gonadotropin-releasing hormone agonist succeeded to preserve fertility in young woman.

BACKGROUND/AIMS: Direct endoscopic pancreatic necrosectomy is increasingly being utilized to treat infected or symptomatic walled-off necrosis (WON) located close to the stomach or duodenum. Laterally-placed WON has traditionally been treated surgically. We evaluated a less utilized technique of sinus tract endoscopy (STE) for symptomatic laterally-placed WON. METHODS: Two hundred seventy-six patients with acute pancreatitis admitted in our hospital, 32 had symptomatic or infected WON requiring intervention. Of the 12 patients with laterally placed WON, 10 were treated by STE. STE was performed with a standard adult gastroscope passed through a percutaneous tract created by the placement of a 32-Fr drain. RESULTS: Ten patients (7 males; mean age, 43.8 years) underwent STE. Mean number of sessions was 2.3 (range, 1–4), with mean time of 70 minutes for each session (range, 15–70 minutes). While 9 patients had complete success, 1 patient had fever and chose to undergo surgery. Two patients developed pneumoperitoneum, which was treated conservatively. There was no mortality, cutaneous fistula, or recurrence during follow-up. CONCLUSIONS: Laterally placed WON can be successfully managed by STE performed through a percutaneously placed drain. Details of the technique and end-points of STE require further evaluation.

ABSTRACT

Dental origins are a common cause of facial cutaneous sinus tracts. However, it can be easily overlooked or misdiagnosed if not suspected by a surgeon who is not familiar with dental origins. Cutaneous odontogenic sinuses are typically nodulocystic lesions with discharge and are most frequently located on the chin or jaw. This article presents two cases of unusual cutaneous odontogenic sinus presentations, as deep dimpling at the middle of the cheek. The patients were undergone surgical excision of sinus tract and dimpling immediate before and after treatment of causal teeth and the lesions resolved without recurrence. Surgeons should consider dental origins of facial dimpling lesions with discharge and provide appropriate treatment.
